  • Tier 2 examples include Follicle Fix, Hair Revive, Power Hair Solution, Power Hair Solution Plus, Raven Solution, and Women’s Scalp Solution.
  • One standard 30-gram customized skincare formula every 90 days for acne, rosacea, pigmentation, texture, anti-aging, or hormonal skin concerns.
  • One standard sexual-wellness pathway. Male options may include sildenafil or tadalafil. Female options may include Euphoria Cream O/S, Petal Revive, Vaginal Rejuv S, or oxytocin mini troches.
  • One standard one-vial wellness therapy every 90 days: methylcobalamin, L-carnitine, Lipo-Mino 10 mL, or glutathione injection.
  • One virtual urgent-care visit and 10% off eligible peptide treatment cycles.

Included Care

  • Complete clinically indicated hormone optimization, including eligible multi-hormone regimens, routine follow-ups, and one standard annual hormone laboratory recheck.
  • Tier 1 and Tier 2 hair restoration together, or one Tier 3 customized oral therapy such as Cedar Oil, Ivy Oral, Willow Oral, or another approved customized combination.
  • One standard 30-gram customized skincare formula every 90 days, with access to approved premium upgrades.
  • Standard or expanded sexual-wellness options, including eligible Olympus formulations, VAST, vardenafil troches, bremelanotide injection or mini troches, and eligible female topical or vaginal therapies.
  • One standard or advanced wellness therapy every 90 days. Advanced options may include the Lipo-Mino Advanced Protocol or NAD+.
  • Two virtual urgent-care visits during the membership year.
  • 15% off eligible Front Range Longevity peptide treatment cycles.
  • Routine clinically required follow-ups, annual comprehensive treatment-plan review, injection supplies when applicable, and highest-priority portal support.

Private Reserve enrollment is limited by provider capacity. When enrollment is full, prospective members may be placed on a waitlist in the order inquiries are received.

Effective-Date Policy

When Your Membership Benefits Begin

Your membership benefits begin on the effective date of enrollment, after the membership agreement is completed and payment is received.

If you enroll during a consultation, that day’s eligible consultation may be included as the first consultation under your membership. From that date forward, clinically appropriate consultations and follow-up visits related to services included in your selected membership tier are included.

Consultations, laboratory testing, medications, or other services purchased before the effective enrollment date are not retroactively included and are not eligible for a refund, credit, or reimbursement.

Membership coverage begins at enrollment and does not apply retroactively.

What “All-Inclusive” Means

  • Benefits begin on the effective enrollment date.
  • Clinically appropriate consultations and follow-up visits for services included within the member’s selected tier are covered prospectively.
  • Services purchased before enrollment are not refunded, credited, reimbursed, or retroactively converted into membership benefits.
  • Membership includes the clinically appropriate treatment pathway and standard quantities described for the selected tier.
  • Medication selection, dose, route, visit type, visit frequency, follow-up cadence, and monitoring are determined by the treating clinician based on clinical appropriateness and applicable requirements, including required video visits for testosterone therapy.
  • “All-inclusive” does not mean unlimited medication quantities, early replacement prescriptions, medically unnecessary visits, or patient-selected therapies without clinical indication.
  • Laboratory testing specifically included in the selected membership tier is covered as described. Expanded, specialty, or additional testing may be billed separately.
  • Benefits remain subject to the signed membership agreement, payment status, applicable law, pharmacy availability, and provider capacity.

Services Billed Separately

  • Expanded or additional laboratory panels
  • Unrelated complex medical visits or extended appointments that fall outside the selected membership benefits
  • Additional medication quantities or vials required by higher prescribed dosing
  • Premium GHK-Cu, short-BUD, and unusually high-cost refrigerated formulations unless specifically included
  • Monthly refrigerated nasal sprays and non-formulary medications unless specifically included
  • Weight-management medications, including GLP-1 medications
  • Eligible peptide treatment cycles after the applicable membership discount
  • Additional urgent-care visits beyond the number included in the selected tier
  • Expedited shipping or replacement medication, when applicable
  • Outside-pharmacy charges

Concierge memberships are annual programs. Benefits are dispensed at clinically appropriate intervals and remain subject to payment status, applicable law, pharmacy availability, and the terms of the signed membership agreement. Membership is not health insurance and does not replace primary care.
